Hospice Palliative Care at Marianhill
Marianhill’s Complex Care program provides client-focused assistance for a predetermined length of stay, promoting independence and optimal function so the client can return to their place in the community.
An interdisciplinary team works with the client and their family to develop a plan of care including treatment goals, treatment plans and discharge planning to identify and implement community support. This plan is evaluated by the team and client on a weekly basis. Health education towards resuming everyday activities and self-medication are important aspects of these treatment plans.
Admission
Upon admission to the complex care unit, the client participates in an interdisciplinary assessment process that incorporates physical, functional, psycho-social and nutritional needs throughout the continuum of care towards discharge.
